    Machka is correct.

    Also, TENS is pretty common. I even have my own TENS unit at home. Runs off a 9V battery.

    There is no guarantee that a doc knows what he/she is talking about. The more specialized they are, the greater the chance is that they do. I look for orthos that specialize in sports injuries and get at least two opinions for any significant issue.
    Ride more. Fret less.

    I have something very similar. Kneecap is off to the outside and not aligned. I think my issue is less severe than yours as I do not have much pain when sitting or walking. I also went to a sports doctor, had x-rays and am seeing a PT. So far, the thing that seems to be helping the most is using a foam roller on the IT bands. I'm also doing step downs (stand sideways on a step and lower the other leg bending the knee on the step and then straightening) and working on a more circular pedal stroke when I ride. It's only been a couple weeks and I haven't done any long rides but I'm noticing a lot less soreness and pain when riding.

    What unit are you using? I notice that Hammer is marketing 2 units:

    I read everything you post here. Thanks. Actually my VMO doesn't look much weak. I am generally a weak guy but I have a good definition on VMO. But it can still be relatively weak comparing to VML. I also think that glutes have a big role on patellar tilt but I didn't discuss that with my doctor (if I do that they think I am trying to be a smart ass and interfere with their job). I have tightness in glutes area and tightness means shortening muscles and pulling the connected muscle groups. I have also tight quadriceps. So, stretching the opposite muscle groups is as important as strengthening the weak muscles.

    It works well enough for my needs.

    You have chondromalacia? As well as patella tilt? Where do you place the pads? My nurse wants to place the pads on the two sides of the kneecap and we set the frequency to 100-110. It is sometimes shaking my VMO and applies a nice pressure from the sides. We also place hot towels over the legs. Today my legs were like a fried duck after the session (40 mins). Would it be a good idea to place one pad on the quadriceps tendons right above the kneecap? Because I have most of the pain on this area..

    I found out that my quadriceps are really tight. Maybe this is the real cause of my patellar tilt. Try this stretching exercise and see if your quadriceps are tight like me but be easy on your quadriceps tendons. Mine are quite sore because of the inflammation so I can't do this exercise as long as I want:
