Cycling Accident Survey
My local bicycle club, New York Cycle Club, is conducting a bicycle accident survey. the results will be published as part of a cycling risk assessment study. Please consider entering one or more accident survey responses. Thank you.

Problem. The survey seems to be stuck in a loop. Every time I choose the answer "New York" it just drops me back into the same page.

Not tying the responses together is less cumbersome, less time consuming and permits one to enter responses annonomously, which many will find appealing. You can find more about the survey's design considerations at the nycc.org's message board discussion.
cc_rider: What web browser, web broswer version and operating system are you experiencing such trouble? This web standard compliant survey has been tested using various versions of IE, FireFox, Netscape, Safari, Opera, AOL web browsers on the Mac and Windows PC. The New York selection is to keep out automated responses, i.e. "spam bots".
