The usual course of action is to pound the existing Star Nut down far enough that a new one can be installed.

Not worth the effort.

I'd just get a new one, drive the old one down or with some pliers pull the old one out.

You could try pounding the buggered star nut all the way out of the bottom of the steerer tube, and then see if you can get the remaining peice of the bolt out. But that is more work than the situation requires. Just get a new star nut and bolt and be done with it.
And use a torque wrench.

I usually hammer old star nuts all the way out. It just bugs me to know that there's another one down there. Plus, if you bugger another one, it's easier to get a single star nut out but nearly impossible to get out 2 of them

can you clamp a needle nose locking pliers on to the broken bolt and spin it out?

So NOW you know that the top cap is only to preload the headset and there is no reason to apply so much force, RIGHT**********

Makes you wonder about the condition of the OP's headset.

I've heard of some people trying to load the bearings without loosening the stem first.
Same end result.
