Rear Wheel locking when turned

Hey, I'm new to maintaining my bike, anyways I decided to pull it apart and grease the bearings, now that its back together the wheel locks up when turned one way(in the forward direction), then loosens when turned the other, anyone have any ideas? I'm not sure what to look at, but have pulled it apart and reassembled hoping to spot the problem...

1) Verify this happens with the wheel off the bike
2) If it is, pull it apart an verify correct # of bearings installed
3) Please tell us what wheel it is

My guess is that the cone is not locked to the lock nut and is turning with the wheel.
When adjusting the axle, you need to lock the cone against the thin nut with a cone wrench.

Damn, you're awesome, I *thought* I had a bearing fall out, so I put in an extra...that was the problem. THANK YOU
