How Worried Should I Be.....
....about this back bent hub thread?
Yesterday, I put on a new rack (an Axiom Streamliner Road DLX) and due to eyelit placement had to mount it on the quick release. Today I noticed that the rear hub's thread is bent on the right-hand side.
Have I mounted it incorrectly? Should I return it and replace it with one that mounts on the eyelits? And ought I get a new quick release thread?

Yes, you mounted it incorrectly.
QR skewer is bent. Buy a new QR (they are pretty cheap). Don't mount anything under them, they have a really thin rod that is not designed for any load. Hub looks fine to me. Mount your rack to the frame of the bike. If you don't have mounting braze ons, then buy some P clamps.

AWESOME. I've been looking for something like 'P-Clamps' for years and I am soooooo thankful you've suggested them. After looking them up on the web I was ecstatic such devices exist! I'm probably pretty over-excited about this small wonder and will obtain some tomorrow (off to work today!)!

What's it look like on the other end of the axle, at the other dropout?
Would p-clamps work? Depends on what kinds of loads you carry on the rack, and maybe on the strength of the p-clamps.

The other side was unaffected, and p-clamps ended up not working out because of the location of the back fork in relation to the drop-outs. What ended up happening was I found a replacement skewer which was properly sized to cover the added rack mounts- as the other one caught the mounts on one side only, thus bending it. It is still mounted on the quick release as there is no other option and at this point I can't return it.
Right now the loads are rather small- 25lbs at the most with the stated load capacity being 110lbs. I do plan on touring sometime sooner rather than later and is the reason why I purchased a heavy duty rack.
