How do I choose what spindle size to get? How is it determined from the crankset?

What cranks do you have? It depends on your crankset. For track it goes a little something like this (cue music):
Sugino 75: 109 matching symmetrical BB. Acceptable variations: 107BB, 110BB shimano.
Campy: 111mm BB
Ah, what the hell am I doing, let me just refer you to John Dacey at Business Cycles, he has the lowdown:
If you're running a road double then you might want a 110-115BB depending. The Sugino RD's run a 103BB.
