Level Hub Axle Replacement?

Anybody here know how to swap a Level Hub axle? I've got a busted axle on one side, right where the spacer is.
It probably happened because the stupid LBS I bought it from didn't include the bolt attachment washers, putting a lot of extra stress on the axle, which Level has also improved since this hub was made. I've got the replacement parts. Level's rep was supposed to send me a PDF of the instuctions, but he is doped up on percocet, as he was tagged fairly hard by a car this week, nothing permanent done to him, fortunately.
