I just can't do it
no matter how hard i try i just cant get the last bit of slippage out of my cog. ive used plenty of grease, ive rotafixed it, ive used cheater bars on the lockring, ive tried everything. i cant ever get it tight enough. is it my cog/lockring/hub set up? ive heard that if the cog isnt thick enough then the lockring wont ever touch no matter how tight it is. could that be the case? i have a DA cog(1/8th) and DA lockring and an iro hub.
ive done successful set ups on suzue-slps and quando hubs so i know its possible and that im not completely inept.
does anyone have this exact same set up and no problems? should i get an eai cog? what the hell is the problem?

The cog thread area might be too narrow. Either get an EAI cog (wider thread area) or try finding a little spacer that fits betw. cog and lock ring to get it tight. Might work.

I would take it to a shop and have them check it out.

You might be able to get a 1mm spacer at your LBS. I tried searching online but couldn't find anything. It's such a cheap part that I imagine not many online retailers are going to stock it.
If that doesn't work, then try an EAI cog. I have one on my IRO hub w/ the Formula lockring and no problems yet.

sounds like you stripped the hub way back when you first installed it.
Formula/DA/DA combo here with no slippage.

I ran a dura ace cog and lockring on a Formula hub (which is what IRO hubs are) for a while and had no problems.
What are you using for a lockring spanner? I personally recommend a Park HCW5 BB spanner. The three-prong end works great on my dura ace hub lockring just by chance. One tooth engages the notch in the lockring, and another makes a fulcrum 1/3 way around the ring. No chance of slipping out of the groove.

this is going to sound really stupid, but are you sure it's slipping? For a while I though I was stripping out my hub, but I was just unweighting my back wheel so much at times that it totally lost traction.

i didn't think snapping a lockring in two was possible either, 'til i torqued an old surly a little too far with a DA tool and SNAP!
...that said, i've had no slippage problems with an IRO hub, DA cogs and the newer surly lockrings (with the additional thread). rotafixed, a 400-lb. gorilla skidding down lombard st. couldn't move them a mm.

Tangentially related: most shops will go "huh?" if you ask for a track cog spacer, but everybody has bottom bracket spacers, which are the same diameter. Handy for fixing this problem and dialing in chainline. See the sheldon article for uber-precise cog width measurements.

Check which way your lockring is oriented. I was using a formula lockring with the beveled edge IN (installed by my LBS). Seemingly unfixable slippage problems. Then I flipped the lockring so that the larger flat side faced the cog. No problems since. Not sure if DA lockrings are beveled.
