chain issue

Okay so I went to skid and my chain just didnt feel like staying on anymore and I was forced to slow the bike with my foot on the rear tire. Im just happy I wasnt stopping for and emergency because I would have been toast. What do you think was the issue (what are the common reasons a chain just comes off). The tension seems to be right on the chain so whats up?
