In my pocket... doesn't really bother me for some reason. Speaking of keys I've got Surly's wrench/bottle-opener on my keychain. It's pretty dope. Although honestly the bottle opener takes a little getting used to.

i have a sweet bottle opener from a bike shop in moaab. also some headset spacers and a couple of metal keys so it makes a ton of noise

I don't trust carabiners on a belt loop. I've once had one snap and luckily heard them clink in downtown traffic. I just throw the keys in my bag now usually.

I carry just one or two essential keys. I keep them, my id and some cash in a small leather wallet I can either keep in my small wallet or else in my petit sized handlebar bag that fits below the handlebars.

This is a good post. And it does suck. I'm trying to engineer (and by engineer, i mean look really really hard for) some type of "smaller than a messenger bag", but "bigger than a pocket" type dealie to keep all my necessities in. In some neighborhoods I keep them in my right hand with the keys protruding between my fingers if I am off the bike. Otherwise in a pouch on my pannier.

The key will stay out of the way if you wear it farther up your arm (past the elbow).

hip bag. they're like fanny packs but not for old people and not neon in color. reload, girlbikedog make them.

Caribiner on the belt loop (like most everybody in BFSSFG) for the most part. If I'm wearing athletic shorts, I clip it onto the d-ring of my courier bag.

You could cut the top off a drinking bottle and put that in one of your cages and use it to carry stuff, shirt, wallet, keys etc.
