stem issues/steerer extenders

i just finished a lengthy tour in reasonable comfort using a 90mm/115degree thompson elite stem holding my moustache bars on my surly crosscheck. my steer tube is completely uncut, so the bar height is at maximum. i still want to get them up and back a little more, but i feel that if i just get a shorter stem, i'll lose too much height.

are there any stems out there with steeper angles that aren't of poor quality? (dimension)

are clamp-on style steer-tube extenders reliable and strong enough for fully loaded abuse?

any suggestions would be great.

I am using a clamp-on steerer extention on my tandem. I have had zero problems and the added height makes for a more comfortable rides. As far as using it on a loaded tour, I can not imagine why it would not work. I am 6'4" and 240lbs, so I put a lot of stress and pressure on my handlebars, probably akin to a touring bike with front loaded panniers.

Most generic stems are available in much steeper angles, but I don't think they're of any inferior quality. (But of course, nothing compares with Thomson's craftmanship.. ) I would try one of those before going the extender route, although there's absolutely nothing wrong with using one of those either.
