hill climbs on the trainer

I have read that hill climbs on the trainer can be simulated by raising the front wheel off the ground say about 4 to 6 inches. My question is do you stop and get off the trainer to place and remove the block during the workout? Do you do this 5 or 6 times to simulate 5 or 6 hills?

Also, are the same leg muscles used on the trainer of you are actually riding in the normal position (hand either on the hoods or in the drops) vs. sitting up as you would to look at a magazine or book or the like. Thanks.
