Medial knee ache

I'm returning from patellar tendinosis this week, from mashing too much on high resistance, and I'm doing some spinning my trainer to see things are going. The pain has been improving a lot this week, until lately, where I've begun to notice an ache on the inside of my knees. I'm pretty sure this isn't my tendinosis acting up. Anybody have any idea of what it is? I haven't had any traumatic incident like a blow and, I'm spinning at 100+ RPM, so I don't think it could be an MCL tear or menicus. FYI, I wear orthotics for flat feet, and I have tended to internally rotate my legs/feet before.

One problem you didn't mention is chondromalacia patella. That can lead to medial knee aches. (At least it does for me!)

consider easy fixes:
over gearing
bad pedaling technique ('pedaling w/ your knees')
cleat too medial
