They make regular pedicabs, normally with two wheels in back and one in front. They also make pedicab trailers which are used behind a regular bicycle. The regular pedicabs are more stylish and preferred, the trailer versions are cheaper.

New pedicabs normally come with 21 speed gearing. They do have electric assist as an option. Which was used in your case, I don't know.

I think part of the trick of using one would be good gearing, but part of it is just going to be a good set of legs and lungs to make it go. And if a mountain bike has adequate gearing to get the bike and rider up a 30% grade, then it could get a pretty good load up a 10% grade.
