I'll lay this to rest once and for all for the benefit of anyone else with a similar problem. I swapped the middle chainring out for TA 40 tooth chainring. No more noise, and the thing runs beautifully now. The shift from the middle to the outer ring is not quite as crisp, but I spend so much of my time in the middle ring anyway. I can live with that.
