Around here (SF Bay Area) it varies. The main trail through my town is administered by the local regional park district and has no indications to separate users by direction or bike vs. pedestrian. OTOH, the river trail between Sacramento and Folsom has instructions for runners to use the gravel next to the pavement. So does the bike and ped path on the new part of the Bay Bridge (south portion is ped only, and the bike part has a dashed line separating west-bound vs. east-bound. A nearby state park has separate ped and bike paths running parallel to each other.
