Top/front of the knee often means that the seat is too low and/or rpm's too low. However you'd think that there would be a problem with both knees in that case.

However I just found out that I have a leg length discrepancy and put in 1/4" worth of shims under the cleat on the shorter leg. It allowed me to raise my seat 1/4". I probably could go to 1/2" of shims, but I'm going see how the 1/4" works first. So far so good.

Another issue could be the cleat position on that side. Possible it's not allowing you to use all the float in one direction.

That "inside" part worries me a little. I have pain on the inside, but it's from torn cartilage from the evils of running years ago. But it's more near the middle of the knee and not the top.
