The photo doesn't clearly show any failure that I could comment on. It does appear that the slot in the ferrule stop is perhaps pried apart, but did it just split down the middle?

One good thing about brake cabling is that one can apply far more tension to the cabling when the bike is stationary than it is likely to encounter while riding.
Unless wet weather causes one to have to apply very heavy force to the lever...

Can you take a picture with the cable and ferrule un-hooked from the hanger? Where on the hanger is the failure?
