Stop by ortho and talk them out of about a foot of the stockinette that is used under casts. The stretchy knit material comes in tubes of different diameters and they should have some for thigh size casts. Just get about 12" of it and fold it on itself to create a 6" double-layer. Put it on like a skull cap and all but the very top of your head will be nice and warm, ears included.
