A lot of new handebars have a 31.8 clamp diameter, but you can still get 26.0. Look at Soma HWY One bars.

I also agree that a Nitto Technomic stem can go a long way to making a bike comfortable for an aging back (I've got Technomics on three bikes), but they're really tall and won't insert far enough to sit anywhere near as low as what you have pictured.
