Yea, the internal anchor at the hub is hard to deal with wearing Mittens..
consider a Sturmey Archer 3 speed?.. external chain shift..
I'd use 2 small nuts on the shift chain threads, block them together to stay fixed in one spot
then the tubular nut the shift cable is attached to should screw back on to the same adjustment
