It was the left crank/spindle interface after all. I pulled out the cap, pinch bolts, and crank, wiped everything down, and reinstalled, putting waterproof grease on the spindle and pinch bolts and evenly torquing the bolts.

It's dead silent now. I can't remember the bike ever being so quiet and smooth before. I should keep up with maintenance more often!

I also noticed my Park tool for the cap has a 8mm hex fitting, so it could be used with the torque wrench next time. Nice feature!
