Rim Damage from Lacing Machine?

Sorry my phone takes terrible photos ever since it got run over by a car.


This wheel came with a new bike purchase about 4 years ago (Trek urban commuter). I'd never taken the rim tape off but was having some puncture problems and thought I would do it now. When I got the old stuff (the hard plastic kind) off I noticed two spots like this, where it looks like a nipple driver or something missed the spoke hole and dug into the aluminum before finding the right spot. The gouges were a little rough so I filed them down with my Dremel before putting on the new Velox. (seems to be working so far)

Did I figure right? Wheel lacing machine error?
Or is it something else?
Ever seen anything similar?
