With the liner rubbing against the tire.
It happened on the front wheel as well, on the side.
I had to cut the liner to fill the hole and prevent the tire from blowing out, and riding with a lower pressure. Thankfully, it was only a couple of days from the end of the trip. 349 tires are only available in major cities, where Brompton retailers can be found.
Get Marathon Plus tires, even though they're heavier than Kojaks.
