Did you use cable ferrules inside the Ergo handles? The newer Ergo handles specify to not use ferrules but the older ones may use them.
Did you cut all your cables squarely and use ferrules where they should be used?
Was the Ergo handle rebuilt properly? When I assembled my downtube Syncro shifters I put in one of the washers flipped the wrong way. It shifted fine for weeks, then completely failed to hold any gear suddenly on a ride. Apparently everything was tight when I put it all together but eventually some extra play in the assembly resulted in the ratcheting mechanism no longer working.
