how to mount cleats in shoe? i'm dumb.

OK, I bought Shimano 520 pedals, and Nike Kato shoes for my new bike.
I didn't think to ask at the store how to mount the cleats to the shoes, as I assumed it would be obvious.
I've tried googling for directions, but can't find any.

I unscrewed the removable part of the sole. I have two different screws, the cleat, and an oval ring. How do they go together? I feel so dumb, I just can't get it to work.

There are four holes in a movable plate in the sole. I positioned the cleat over one of the rows of two holse, places the oval ring inside the cleat, and put both screws in. So far so good. Then I wanted to see if it worked, hopped on, was easily able to click the shoe in, but then can't remove it!

I kept turning my foot, but it wouldn't come off. I took the shoe off, and looked--the cleat is staying in the same place, and the shoe moves, almost 180 degrees, until it can't go any further.

What the heck did I do?
