It's difficult to know what happened in these situations from short excerpts, but its sounds like a right turn hook.
Not to absolve the truck driver, or cast blame on the deceased, the incident does point out that we need to be very careful around trucks, and not stay in the "No Zone" i.e. the area the driver can't see. Whether that's what happened here or not, it's a reminder to be vigilant around trucks.

This is a cold reminder to us all that cyclist vs. anything else on the road, regardless of fault or who's in the "right" = cyclist loses. Be careful out there, when you're riding a bike, you're very exposed and unfortunately, humans are very fragile.
