Are helmets required?

Okay, first off, I'm not trying to start a debate about whether or not they SHOULD be required. I just want to know whether or not they are. In almost all the pictures and footage I see of pro racing, they ARE wearing helmets. However, I seem to remember in the TDF this year, most (all?) riders weren't wearing them on the ITT up Alpe d'Huez. I understand why they wouldn't want to wear it (weight, cooler, etc.), but are they not required? Are they allowed to leave them off in select stages? Maybe the ones (like an UH TT) where a fall is not likely?

When the stage has an uphill finish longer 5km I think it is? they are allowed to ride without a helmet.

Correct, as long as the climb is longer than 5k, and finish is on that climb.

To clarify. As noted above, on a stage with an uphill finish, riders can only get rid of their helmet on the final climb. The rest of the stage they have to wear it. If you watch you'll see a kite on the side of the road indicating when they can take it off.
