I had a really bad incident 2 years ago while riding a geared mountain bike in heavy traffic in Paris. I was out of the saddle going at a good clip, with a big parcel truck right behind me when my chain slips off.
It wouldn't have been too bad except for the fact that I was out of the saddle and my testes come down
hard on the top tube. Fortunately, I'm running platforms and put my feet down and Fred Flinstone it, trying just to stay up and get out of the way of this huge @ss truck behind me. Alas, I lived. My brother who was riding with me felt the need to say "You almost just died, you know that right?". Check your drivetrain often, gentlemen.
