I see absolutely nothing wrong here. When you are in your largest cog in the rear and largest chain ring in front your RD should look just like in the pictures. As for the chain falling off when cross chaining I wouldn't adjust anything, that is also normal, just stop cross chaining and you will be fine. Same goes for the sluggish shifting in the rear when cross chaining. When cross chaining, your gears they will obviously not work perfectly, which is good because it teaches people to find a different gear instead of rapidly wearing out their drive train.
