More silly questions from a PBP first-timer: What does the "medical certificate" need to say? Can it be jotted on a prescription pad? Is letterhead necessary? Someone have the magic words handy that worked for them in 2003?

I can see a lot of docs in the U.S. -- with our litigious society -- being pretty hesitant on digning off on these things. Anyone run into that problem in the States?
