Fix that grinding noise?

We purchased a Cannondale MT3000 last year and have enjoyed the world of tandem riding. Early this season we developed grinding noise that would only start after 10 - 15 miles. To make a long story short, after a couple of months of replacing bottom brackets, chain rings, and chains, only to have the noise get worst. I'm hoping I've got the problem nailed. I was able to pin point the noise coming from the hub. I took the bike to yet another bike mechanic that thought I was crazy because the hub did not have any classic signs of going bad, but he agreed to rebuild it. He had to contact the manufacture (DT Swiss?) of the Coda hub because he did not know how to tear down the hub assembly. They told him that the freewheel assembly, if not maintained properly, was know to present a grinding noise.

I don't have the bike back from the shop yet, but I'm very hopeful. I thought I'd post my find because after searching many threads for an answer, I never saw this explanation.
