I have Keo's. Recently on a group ride a rider commented to me about my ankle turning out on my pedal stroke. I looked down and I was moving my ankle out every stroke. After the ride I looked at the cleat and it was worn down over 1/8" on one side causing my foot to follow the cleat's contact angle. New cleats made noticable difference.
added later: I had about 3,500 miles on the cleat.
