Shift all the way and then click up once. Adjust until the derailleur and chain line up to the 13 using the barrel adjuster. Check that the chain runs quietly. Click up one to the 14. Does it align properly? Go the other way. Does it go down to the 12 properly? If it won't go to the 12, adjust the limit screw so it falls in. If none of this works, something is probably bent. Are you sure you are using shift cable housing with the parallel strands and not spiral brake cable housing?
This. Following your advice 12,13 and 14 are good, but 15 gets skipped.

With the chain on the smallest cog flip the small shift lever several times to make sure all of the cable is out of the shifter. There should be no slack in the cable.
Done, everything appears to be correct. If I remove the cable from the derailleur I get 8 clicks in both directions. When I hook the cable up, I only get 7. The derailleur and cogs are all lined up for every click, so that's last click is making a huge jump causing the 13 to be skipped.

Cable and housing is all brand new.
So to summarize: With cable disconnected I get 8 clicks from the shifter. With cable connected I only get 7. I'm at a loss as to what could cause this.
