What shifters again? (maybe I missed this earlier)
Have you tried a new middle ring/crankset? A heavily worn middle ring could want to hang onto the chain enough so that it won't easily derail down to the granny. Clear, close-up pics of the area could help this diagnosis.

That was my next thought - to replace the middle chainring.

I like Surly's stainless steel chainrings for their Long Wearing ..
they dont have the fancy shift gates and lift pins, but friction shifting that is less Important.

Thread revival: Without having to run the low limit screw way out so the front derailleur is further inboard than would be proper any other recommendations? This is a persistent issue on MTBs this vintage it seems. The middle chainring isn't worn enough to warrant replacing.

Don't be obsessed about "proper chain line".
It's only "proper" on very few gear combinations anyway.
IF you can get it to shift by backing out the L screw a bit more, than you've wasted months.
IF that doesn't do it, get the next longer BB.
