Creak from stem

I’m getting creak from my stem. I can’t tell if it’s the faceplate or fork bolts. Its obnoxious climbing out of the saddle and frightening while sprinting (note – I sprint from the drops so it’s not coming from the brifters). Its an ITM stem rebranded by Colnago and a Colnago Star carbon fork. I believe the bars are ITM but don’t remember. They were sold as a set with a C40 I purchased in 2003. All four bolts are tightened to the manufacture’s specification of 6 Nm. I’ve inspected all the joint areas and can see no damage or cracking.

Anybody have an idea for making this creak go away?

Put some of that carbon fiber assembly gel on it and see if that helps.
