New chain and new cassette. Or if you have just changed one change the other.

That's not very much information to work from. When is the last time that you had the derailleur checked?

I have had the bike maybe a year or longer. I don't believe the derailleur has been checked. The last couple of times I have gone out for a ride, the drive train will skip itself.
Its a 9x2 mix of XTR, Campy Record, regular Shimano with Gevanalle Shifters.

If it was working for most of a year, and you started getting problems with 'ghost shifting', the most likely culprit is cable stretch.
Get the bike situated so you can be behind it and eyeballing straight down the chain as the cranks turn slowly, and work the rear derailleur barrel adjuster until you can see that the upper pulley is directing the chain to be precisely centered on the correct cog.
Alternatively, shift to a middle cog, screw the barrel adjuster OUT 1/4 turn at a time until the chain is starting to touch the next smaller cog. Then screw the barrel adjuster OUT 1/4 turn at a time (counting the turns) until the chain starts to touch the next larger cog. However many quarter turns you counted, screw the barrel adjuster back in half that many, it should be well centered.
If that doesn't work, then as mentioned above, lots of things could be going on; chain/cassette could be worn, derailleur and/or hanger could be misaligned/bent, etc
