Stem/Headset Cap Stuck
 

My stem/headset cap is stuck. By that I mean, I can spin, and spin, and spin the bolt but the cap never comes loose. I suppose the star nut is just spinning inside the steerer tube? Other than that I have no idea why this could be happening. Any ideas? Better yet, any ideas on how to remove the cap?
