What might have caused this failure in a BWR hub?

I have a 6-speed Brompton with the rear BWR hub. Recently, the high gear stopped working. Upon disassembly, we found a broken part. It is like a specially shaped washer with "flaps" that ride over the pawls, guided by tabs that ride over a groove.

I couldn't figure out what might have caused the flaps to catch and break. Does anyone have an idea?

It's called an actuator plate. In a non coaster brake hub, it performs a non vital function. You can put the hub back together without it, but you may find that you can't back pedal in 3rd.
The loose broken parts probably prevented the clutch from engaging on the planetary carrier which would cause 3rd gear to not work.
In a coaster brake hub, that part is vital.

This is very useful, thank you! I see that your YouTube channel has a video called "Sturmey Archer mystery part" which describes precisely the actuator plate.

I hope you don't mind an extra question:

You say that the hub will work without the plate, but backpedaling won't be possible in 3rd gear. If I'm unable to find a replacement part, should I mount the plate without the flaps? I.e. does it have a function as a spacer, or will the hub be fine without the plate altogether?

No, it's the flaps that serve the function of retracting the driver pawls when back pedaling. There would be no point in putting the other piece in without it.
I have assembled hubs without the plate, and as I indicated, the only issue will be that you can't back pedal in third gear.Other than that, it will be fine.
