I don't see mention that you've looked at the shifter or cable guide (if your bike has one), why not? Seems those two points are the only remaining things to check. Or too short of housing somewhere or too long, causing extreme cable bending/binding.

Does this happen on all the chain rings? I read your post and was not clear on that. I was wondering if you are cross chaining when this happens? Or? If you have a light rear derailleur hangar it may be bending.

Here's where we're at:
Shift cables and housing replaced.
Shifter flushed with WD-40, also with degreaser, and then lubed.
Derailleur hanger alignment checked yet again.
New chain yet again.
Derailleur adjusted (H and L limits, B screw) yet again.
And recall that the RD and Cassette have both been replaced for other reasons since the shifting went wonky.
I'm gonna bite the bullet and buy new shifters - they're only available as a pair. At least I can get them from a retailer that will allow me to return them if it turns out to not be the issue...So I will carefully install the right (rear) shifter, try it out, and see if it works. Will post here after I have tried it.

I've had this problem with a road bike for a few years, and have asked a couple mechanics to look into - usually when the bike is in a shop for other work. So far, no one has solved it.
Ten-year old Shimano 10-speed groupset with compact crankset. Bike shifts fine on the top 6-7 cogs (biggest cogs/lowest gears). When I try to shift down the cassette past the sixth or seventh cog (so, trying to shift to higher gears), chain won't drop down to next cog immediately, if at all -- usually I have to click the shifter again, and then the chain will drop two cogs. Shifting up the cassette (to smaller gears) works fine.
Following has been tried:
-Derailleur hanger alignment checked (3-4 times).
-New cable.
-New chain (replaced regularly anyway).
-New cassette (normal replacement)
-New RD.
I've even tried a different wheelset. Nothing works.
Ideas from the BF brain trust?

Also, I can’t recall if you said whether you replaced the bottom bracket cable guide or not - if you haven’t you should do that.
And can you tell us the brand of cable and housing you are using? My LBS mechanics that I have talked to all say the same thing about the current crop of polymer coated “slick” cables - they suck royally. The plastic coating starts degrading right away and they have been known to fail in as little as 500 to 1000 miles. Also did you replace the housing yourself or did the bike shop do it? Could you post a picture of the rear housing loop? And are you certain that the ferrules were installed correctly (housing ends cut square, any burrs filed smooth, proper compressionless housing used and ferrule seated fully on ends of housing?
i had crap shifting with my Dura Ace 7700/7703 9 speed set-up with symptoms very similar to yours. Jagwire LE-X housing, Jagwire stainless “polished” (not coated in any way) inner wire and a new bottom bracket guide restored perfect operation. The guide might look OK and appear to slide OK but lubing it with oil or grease will not cure it. Remove it and look at it - mine had grooves eroded in it from cable inner wire strands that acted on the inner wire in a bad way. Once this little $3 part was replaced things got noticeably better immediately.

Which specific 10 year old group set components are you using? Just curious - does the rear derailleur housing run out the side of the lever or under the bars?

I've now even tried another 10-speed brifter (one which is known to function properly) and the problem remains...so that little BB cable guide is the last item. I'll try it. And if that doesn't fix the problem, I'm gonna just ride the bike as-is, since I have a couple other bikes. It'll become my "foul weather, beat-the-s**t outta it" bike.
