B screw misaligned
On my Shimano RD 7800 the B screw was not hitting the appropriate tab on the derailleur hanger, so I turned it around so that the screw head was catching on the tab. This works most of the time but once in a while it will still slip off the tab resulting in a lot of clicking and clacking when in the small chainring and large cassette cog only. Anybody ever have this problem and come up with a solution? I'm afraid if I bend the tab on the derailleur that has the screw I might break it off.

Make sure the derailleur hanger itself isn't bent.
