Sturmey Archer 3 speed clicking + ghost pedal

I have a 3 speed Sturmey archer and in 3rd gear it makes a clicking sound, it's like a metallic creaking sound coming from the chain guard. (not the pawl clicking)
On the non-drive side I previously went to regrease the cone bearings and found that it had 1 missing so replaced all on that side.

It only happens when I am pedaling and probably once each revolution of the pedals. It doesn't happen in first or second. Seems to happen more when the pedals are under more strain. Doesn't happen at all in 1 or 2nd gear even if pedaling up hill.
It's hard to tell where the noise is coming from because it just sounds like the chain guard.

The bike also ghost pedals. It was worse but I loosened the adjustable cone and now it only grips the pedals for maybe 1 revolution of the pedal then it's fine for a bit, and grips again. only in 3rd gear

If I invert the bike and run the pedals backwards it will sometimes grip/lock the pedal (only in 3rd gear)

Any ideas or has the hub got to come apart?

Ghost pedaling is one sign of too tight an axle bearing adjustment. I set the AWs I service to have a very slight bearing slop if felt at the rim. Another nono is too tight a chain. What cranks does the bike have? I would check both the BB condition and adjustment as well as the crank arm retention Cotters can loosen and/or get worn on the wedging surface and cotterless can have arms loosen on the BB axle. Exactly.
I had one where the ball ring (freewheel bearing) and driver races were in such a bad shape that i caused ghost pedalling.
