Crank cut in 2!!

Yesterday while working at the bike shop I had a bike come in with a crankset problem. The crank arms were no longer opposite of each other. Pulled the set and found the shsft was cut in 2! The internal cable for the FD was routed right over the top of the crank shaft and acted as a saw. No way to change the routing so we called the customer and suggested he take it to a Cannondale dealer.
So anyone ever come across this situation?

I’ve seen other claims of that happening (eg posts on other forums) but not seen it in person. This guy claims it happens if you route the cable below the spindle rather than above, and his bike has a warning sticker to that effect:
