Why does my chain not want to shift?

So if I decide to shift from the largest chain ring to the middle chain ring my chain often complete misses the middle chain ring and slips betweed the middle and largest chain rings. This is getting quite annoying as I will be in middle pedal and then BAM....I can't pedal at all! Any ideas since my LBS is compelety inept?

Adjust the deraillieurs.

As with above, the cable needs tightening so there is more 'distance' between top and bottom.
