Disc Brakes

Hello,
I am geting back into riding. I have a dirt bike motorcycle and I want to get back into shape. Here is my question. How do you tell by looking at a frame if it is ready for rear disk brakes? Can I assume that the front forks have the taps at the bottom for the caliper? How do I tell for the rear disc? Thanks, Tony
