replacing rear shifter cable on Shimano 600 brifters - help!

I've got a new-to-me Shimano 600 8-speed bike, and I'm having trouble removing the cable from the rear (drive side) brifter. Doing the same to the front brifter is no problem, as the cable end exits straight out the side of the lever, but the cable on the right side takes a downwards turn, so the cable end is down in the bowels of the lever.

I've tried "upshifting" and "downshifting" with the cable free of the derailleur; Does it need to be installed on the derailleur in order to be removed?

Anyone know how to get the cable out, and pop a fresh one in?

You have to shift the lever to the most "relaxed" position, that is to the smallest cog. Put some tension on the cable by hand and shift the inner lever at least seven times. You should feel the cable lengthen a little each time until you are in the high gear position. Then the cable head should be visable from the outside of the lever just as it was for the front brifter and you just push it straight out. You, of course, have to pull the brake lever inward (as if applying the brakes) to see the cable head.

If this doesn't work, there is something wrong with the brifter.
