chain skipping around upper or middle ring

I have a hard time to understand wha'ts wrong with my mechanics (dura-ace 9v).

I am tweaking chain tension because my middle rings skips and sometimes miss the change. But when I soften the problems that way then my upper ring skips.

What's wrong?

Cables is the first guess. DA seems to have soft spring so any friction in the cable with housing normally does what yu are discribing.

Is it skipping once you have shifted or only skips when doing a shift?

I'm guessing it's during a shift. If so a lot of things other than chain (or cable?) tension can be to blame. And they are all related to the derraileur cage. The angle and height above the bigger ring teeth it sits on is important and often the sides of the cage need a little bending to tweak in that last bit of fine tuning to move the chain far enough.

Also if you have a left brifter that has the extra positions for trimming are you shifting enough clicks at a time? I don't have one of those style shifters so this last bit is a shot in the dark....
