High pitched sueaking noise from high gear
 
I am hearing a very high pitched squeaking noise from the rear cassette only in the smallest cog and with the large and middle chain rings. It only occurs on the smallest cog and increases in speed the faster I go. I have lubed everything and put it on a repair stand and nothing is rubbing. I can't figure out what is causing it. It only occurs when pedaling, totally quiet if coasting.

Der pulleys. Shoot the bearings with WD40, if it goes away, take apart and grease.

Does it make sense if it is the pulleys that the noise would be there only on the smallest cog?

Yes, because on the smallest cog, the chain will be putting sideways thrust on the bearing. Just hit it with wd40/oil and see if it goes away.
