Are handlebars standardized?

Are handlebars standardized?
I bought a bianchi mixte frame last night. The frame a bit on the small side, but I want to replace the handlebars and once I do that, I should be seated more comfortably.
I've measured the diameter of my handlebars, and I get 25.3mm. I looked up handlebars online and many of the ones I found are 25.4mm. Am I just measuring wrong (the handlebars are 15 years old and might be a bit flattened, or I may not be measuring at the exact right angle), or do 25.3mm bars exist? And if I get new handlebar grips, will any grips fit? I didn't see any measurements on the grips I was looking at.
Secondly, the rings that attach the breaks and the gear shifters are slightly less than 2cm each (.75 inches). In total, I think I'd need about 6-7mm (2.75 inches) of available space on each handlebar to fit both items. Is this pretty standard with most handlebars (drop bars and fixie bars excluded, of course), or do I need to be very careful and obtain measurements before ordering?
Lastly, is there anything else I ought to know before I proceed with this?

25.3mm is within the error margin of 25.4mm, or 1" clamp.
if these are flat bars you are talking about, the narrower portion should measure 22.2mm, standard MTB size. If drop bars they should measure 23.9~24.1mm at the narrow portion.
I don't quite understand the second part. I need more tea.
