How much disassembly for travel/shipping ?

How much disassembly for travel/shipping ?
How much do I need to disassemble a bike to travel with it or ship it ? I was wondering if it as simple as removing handlebars, wheels, seatpost and pedals. If it is that simple I could do it by myself. If I have to remove drivetrain bits, I wouldn't feel comfortable doing it myself, making it unfeasible because the LBS would charge too much.

If you're putting it into the standard bike box that bikes get shipped to shops on typically it's
1) Front wheel
2) Handlebars
3) Pedals
4) Seatpost/Seat

I was wondering if the brake calipers would get in the way.
Also where can I get the box ?
